Nuffield Health, Parkside Hospital | Wimbledon, London| Permanent | Part time 18 hours per week| FTE salary Up to £29,842.00 per annum, pro rata-depending on skills and experience

We are looking for a Rehabilitation Specialist to help us build a healthier nation. We are looking for a Rehabilitation Specialist who will deliver Nuffield Health Rehabilitation Programmes utilising the unique Nuffield Health pathway, championing the Charitable Purpose. This role will play a crucial part in Nuffield’s long-term vision for clinical rehabilitation and will require the Rehabilitation Specialist to undertake remote (digital) and in person facilitation of exercise, emotion wellbeing support, healthy eating, and goal setting to aid the participants recovery. The pathway will involve weekly 1to1s, group exercise classes (virtual and in person), promote goal setting and self-directed activity.

You will demonstrate leadership and influencing skills, be a good communicator, organised and self-motived. As part of this role, you will work collaboratively onsite and offsite, centrally and with our Charity team and our beneficiaries. You will need excellent IT skills and hold a Level 3 Personal trainer qualification which is essential for the role. Ideally you will have previous experience working in a Rehabilitation Specialist position and an understanding of rehabilitation, along with potentially a Bsc (Hons) in Sports Rehabilitation (desired not essential).

As a Rehabilitation Specialist you will:

  • Organise and run a series of rolling 12-week rehabilitation programmes.
  • Facilitate and deliver both live stream and in person group exercise classes, with people that have varying degrees of abilities and physical function.
  • Be able to promote self-directed activity using pre-recorded on demand classes and a journal which will aid in the participants being able to self-manage their condition after the 12-week Programme.
  • Capability to re-calibrate exercises and intensities based on regular improvements demonstrated by changes in both effort and performance.
  • Identify any deterioration of participants and if necessary follow internal escalation processes, ensuring reporting is processed within a specific timeframe and that outcome data is accurately captured, which is essential in supporting our Charitable Purpose and tracking and monitoring participants progress.
  • Engage in 1to1 telephone conversations each week with every participant to offer coaching in terms of setting short term and long-term goals, understanding restrictions, monitoring progress, understanding and offering emotional wellbeing support, and empowering self-directed activity.

Where necessary and depending on which social impact programme.

· Identify suitable community locations (such as community centre, sport centre or local authority gym) to deliver the programme from.

· Complete appropriate risk assessments & site evaluation to ensure location safe and appropriate

· Ensure site is set up & ready to deliver the programme for each session. This includes ensuring equipment stored safely after use.

· Ensure any site-specific rules and procedures are followed

· Recruit participants into the programme, working with site and GPLO’s

· Monitoring Cohort registration/referrals

· Manage the transition of phases and overlapped phases

· Deliver a rehabilitation programme of varying lengths, models (virtual and in person) and specialist areas.

· Facilitate Cohorts of 8 – 13 participants 

· Engage in weekly 1to1 telephone conversations to help set goals, understand restrictions, progress, and offer emotional wellbeing support 

· Deliver live stream classes for the cohorts using MS teams, with people of varying abilities and physical function.(where applicable)

· Deliver in person exercise classes, with people of varying abilities and physical function

· Facilitate individual & groups learning with the aid of the journal and Hub.

· Manage a discharge process when participants have completed their rehabilitation programme.

· Identification of deterioration and further referral, including internal clinical escalations where appropriate.

· Ensure that the participants are working through and completing their support journal, highlighting sections of particular help.

· Ensure that all sessions are scheduled ahead of time and participants receive their sessions plans in advance.

· Deliver programme assessments where necessary

· Use of the correct entry point scale for all participants, offering progressions and regressions for all abilities.
